CDFGC embraces the SPF model as an effective planning process to assist coalitions in development and sustainability. The five steps process will enable communities to build the infrastructure necessary for effective and sustainable prevention.
General Resources
Step 1: Assessment & Tools
Profile population needs, resources, and readiness to address needs and gaps. Assessment involves the collection of data to define problems within a geographic area. It also involves mobilizing key stakeholders to collect the needed data & foster the SPF process. Step 1 also involves an assessment of readiness and leadership to implement policies, programs, and practices.

Step 2: Capacity & Tools
Mobilize and or build capacity to address needs. A key aspect of Step 2 is convening key stakeholders to plan & implement sustainable prevention efforts. Readiness, cultural competence, and leadership capacity are addressed and strengthened through education and training.

Step 3: Planning & Tools
Develop a Comprehensive Strategic Plan. Step 3 involves the development of a strategic plan that includes policies, programs and practices that create a logical, data-driven plan to address the identified problems.

Step 4: Implementation & Tools
Implement evidence-based prevention programs, policies, and practices. Step 4 involves action planning, or the selection of specific policies, programs, and practices and then taking action! Implementation also includes the collection of process measure data and the ongoing monitoring of implementation fidelity.

Step 5: Evaluation & Tools
Monitor, evaluate, sustain, and improve or replace those that fail. Step 5 involves measuring the process and impact of the implemented programs, policies, and practices. It also includes reviewing the effectiveness, efficiency and fidelity of implementation as it relates to the strategic plan, action plans and measures.
